COLOGNE, Germany: Deep caries is the most common reason patients visit the dentist, and one in three patients present with a deep cavity. Pulp exposure is always a risk in cases of deep caries, and so pulp vitality should be preserved at all costs.

When a deep cavity is close to the pulp, accidental pulp exposure can occur during caries excavation and compromise pulp vitality. How can pulp vitality be preserved?
The first way to preserve pulp vitality is to avoid pulp exposure by using the minimal intervention dentistry approach for caries removal, and the second way is to choose the right restorative material. This material must stop the progression of the carious lesion and preserve pulp vitality by stimulating reparative dentine. It should increase the distance between the final restoration and the pulp–dentine system and create a barrier effect. If the dentine layer is very thin, this is especially important for reducing the risk of cytotoxicity and bacterial microleakage.
The ideal material has the following qualities: biocompatibility; high remineralisation, ideally of both affected and infected dentine; and outstanding sealing, mechanical and antibacterial properties.
